University Hospital at Downstate Deploys Altera’s AI Scribe Across Clinical Care

University Hospital at Downstate (UHD), the academic medical center of SUNY Downstate Health Sciences University, has deployed Altera Digital Health’s Sunrise Thread AI to automate clinical documentation during patient visits.

The ambient AI scribe captures patient-provider conversations in real time and generates a structured clinical note inside Altera’s Sunrise electronic health record (EHR). Since the pilot began in May, 50 providers across family medicine, internal medicine, cardiology and the emergency department have used the system in more than 900 patient encounters, Altera said.

The deployment comes as health systems expand the use of ambient AI to reduce the documentation burden on clinicians. Other health systems have also been moving AI scribes into clinical workflows.

UHD serves Brooklyn and is the only academic medical center in the borough. The hospital has worked with Altera as its EHR partner for nearly 20 years.

AI Scribe Built Into Existing EHR

Sunrise Thread AI is designed to operate within the existing Sunrise environment rather than requiring clinicians to switch between separate applications. Providers can record conversations using a mobile device, while the resulting note can be reviewed and approved in the EHR.

Altera introduced Sunrise Thread AI in 2025 as an ambient documentation tool designed to capture clinical conversations and convert them into structured notes. The company positioned the technology as part of its effort to reduce the time clinicians spend on manual documentation.

The approach follows a wider push to bring AI directly into electronic health records. Sutter Health, for example, has also incorporated AI capabilities into its clinical workflows.

Dilip Nath, Chief Information Officer at SUNY Downstate Health Sciences University, said the early response from clinicians had been positive and that demand for the technology remained high.

UHD is tracking the system's usability, transcription and interpretation quality, documentation workflow and potential improvements. Altera and the hospital are also developing customized templates for inpatient and emergency department physicians based on early feedback.

Downstate Expands Use Across Clinical Teams

Mafuzur Rahman, Vice Chairman and Director for Inpatient Services and Chief Medical Informatics Officer at SUNY Downstate Health Sciences University, said the system had given clinicians more time while improving the completeness of documentation.

The hospital also expects the technology to help medical trainees who rotate through different clinical settings. UHD has nearly 1,000 students, interns, residents and fellows who have varying levels of familiarity with its EHR.

Patient consent remains part of the implementation. UHD requires providers to tell patients when encounters are being recorded for medical-record documentation, while patients are informed about AI use in their care as part of the general treatment consent process.

Altera said the recordings and conversation data remain within UHD's existing environment. Clinicians can review notes immediately after an encounter or return to them later through the desktop interface.

The pilot gives UHD a way to evaluate ambient documentation within its existing EHR infrastructure before expanding its use across additional clinical teams. Altera said it plans to continue working with the hospital as Sunrise Thread AI expands at UHD.
