Sentara Health Pilots GW RhythmX AI Platform for Primary Care

Sentara Health is testing GW RhythmX’s AI platform with 30 primary care clinicians to evaluate clinical recommendations, adoption and workflow fit.
Sentara Health is piloting an artificial intelligence platform from GW RhythmX to provide patient-specific clinical recommendations during primary care visits, expanding the health system’s use of AI from administrative tasks into point-of-care decision support.
The 90-day pilot will involve 30 physicians and advanced practice providers (APPs) in family and internal medicine across two markets. Sentara will evaluate clinician adoption and trust, the usefulness of the recommendations and how well the technology fits into existing workflows before considering a wider rollout.
Sentara operates 12 hospitals and more than 400 care sites across Virginia and Northeast North Carolina. The not-for-profit health system reported $14.2 billion in operating revenue in 2025 and has approximately 473 primary care physicians and APPs within Sentara Medical Group.
AI Moves Into Primary Care Workflows
GW RhythmX was formed in 2025 through the combination of RhythmX AI and Get Well. Its precision care platform uses generative and predictive AI to analyze information including patient history, clinical guidelines, care gaps, social and financial determinants, payer requirements and formulary considerations.
The system then presents patient-specific recommendations to clinicians during an encounter. GW RhythmX describes these as next-best-action recommendations intended to help care teams identify opportunities for intervention and coordinate care.
The approach reflects a growing effort by health systems to put clinical AI directly into existing workflows. Sutter Health has also moved AI into its EHR workflows, integrating OpenEvidence into Epic so physicians can access clinical evidence while working inside the electronic health record.
Sentara’s pilot is focused on the cognitive workload associated with primary care. Prasanna Mohanty, Executive Vice President and President for Ambulatory Care at Sentara Health, said the health system was redesigning its ambulatory model around team-based care involving physicians, APPs, nurses, pharmacists and care managers.
“We're looking at RhythmX AI to augment that model by reducing not only administrative burden, but the cognitive burden,” Mohanty said.
The system will also run a control group using existing Sentara tools. A separate pilot of Sentara’s team-based primary care model will run alongside the AI deployment, allowing the health system to assess the effects of the two initiatives independently before combining them as they scale.
Sentara Will Test Clinical Impact Before Scaling
The pilot comes as healthcare organizations expand AI beyond documentation. AI scribes are moving closer to routine healthcare use, with ambient tools primarily focused on reducing the time clinicians spend preparing medical notes.
GW RhythmX is targeting a different part of the clinical workflow. The company’s platform is designed to consolidate information from multiple sources and provide recommendations that clinicians can review during patient encounters.
Sentara will assess whether clinicians follow the recommendations and whether the technology leads to meaningful clinical actions. The health system will also examine whether the system helps primary care teams manage more complex patients and reduces unnecessary specialist referrals.
GW RhythmX previously expanded its platform across Presbyterian Healthcare Services after a pilot involving more than 10,000 patient encounters. The company reported that clinicians identified more than 200 combined HCC and non-HCC conditions and generated 17,000 hyper-personalized treatment responses during the initial deployment. Presbyterian subsequently expanded use of the platform to 200 primary care clinicians.
Sentara said its AI deployments go through a governance process focused on scalability, safety, compliance and clinician oversight. The health system will use the pilot results to determine whether the technology can support a larger deployment across primary care and potentially other ambulatory settings.
The evaluation also reflects a growing focus on measurable operational outcomes from healthcare AI. Healthcare AI's measurable returns are increasingly being tied to patient capacity, including whether technology allows clinicians to spend more time on patient care.
For Sentara, the immediate test is narrower: whether AI-generated clinical recommendations can help primary care teams process complex patient information and make decisions within the existing care workflow.
