Task-Specialized Models Deliver Where General AI Fails in Healthcare

As healthcare leaders chase bigger LLMs, revenue cycle teams are achieving 30% denial reduction by narrowing AI scope and adding rigorous governance layers.
In two months, Cigna's automated denial algorithm rejected 300,000 claims at a rate of 1.2 seconds per review.
A staggering 90 percent of those denials were reversed on appeal. The system was wrong nine times out of ten, yet payers continue this approach because volume and speed overwhelm provider capacity to challenge each decision. This episode encapsulates the crisis confronting revenue cycle leaders: payers are deploying AI aggressively to accelerate denials, while providers lack the capability to respond at similar scale.
The crisis is measurable. In 2025, denial rates averaged near 12%, with many organizations experiencing even higher volumes. By 2025, 41% of providers report that claims are denied over 10% of the time, according to Experian Health's State of Claims 2025 report.
Yet the response from healthcare organizations reveals a paradox. Although 67% of providers believe AI can improve the claims process, only 14% have implemented AI tools. Of the small group using AI, more than two-thirds (69%) say it has successfully reduced denials and/or increased the success of resubmissions.
The gap between proven capability and deployment indicates something beyond skepticism.
The Architecture That Actually Works
The underlying problem isn't confined to healthcare. According to McKinsey's 2025 State of AI report, 78% of organizations now use AI in at least one business function, yet fewer than 6% report generating real financial returns.

Michelle Castillon, Chief Product Transformation Officer at Vee Healthtek, explains the company's approach to AIM Media House. "So there is so much variation across our processes that it's extremely difficult for end users, for human beings to always know all the right rules, right steps and make those decisions," she observes. The company required that authorization prediction models be 95% effective or accurate before deploying into the environment.
Rather than deploying general-purpose LLMs across entire workflows, Vee created task-specialized models for each bounded operation. One model focuses on eligibility interpretation. Critically, models only get invoked when deterministic logic fails. For routine cases where policy clearly applies, the system executes rules without model inference. If the system processes a high-dollar case, it escalates to human review. If a claim involves multiple payers or complex medical necessity arguments, the system routes it to specialists rather than attempting automation. This layered approach means automation only handles cases where risk is low and rules are clear.
Why Implementation Takes Longer Than Anyone Expects
Muthu Krishnan, Chief Digital Technology Officer at Vee Healthtek, explains the technical reality. The company starts with foundational models from open-source and public providers, then specializes them progressively. First, models are adapted to the problem domain. Then they're fine-tuned for the specific client and payer combinations involved.
The constraint isn't model development. "The reason why it takes a long time to get any of these implementations done is because we do not want to regress any of the work that is happening. We have to take the time to make sure that the data and the data model and the process model are aligned with the product model," Krishnan observes in conversation with AIM Media House.
Healthcare data comes from multiple sources. Payer formats vary. EMR systems have different terminologies. Building a unified ontology and taxonomy that maps all these variations takes substantial effort. Krishnan notes that "Developing the product itself is actually quite fast these days."
This inversion would have been unthinkable five years ago. Most organizations assumed models were the bottleneck. They're not anymore.
McKinsey's research on high-performing organizations reveals that companies capturing outsized AI returns treat the technology transformation fundamentally differently from laggards. High performers redesign entire workflows, not just add AI to existing processes.
This pattern appears across industries. In financial services, JPMorgan Chase's Contract Intelligence (COIN) platform reviews commercial loan agreements using models trained on financial documents, not general text. Tempus processes clinical and molecular data across cancer, cardiology, depression, and infectious diseases. The company went public in June 2024 with AI systems trained exclusively on medical data.
The broader market recognizes this shift. Bessemer Venture Partners reports that vertical AI companies, those building specialized systems for specific industries, are growing 400% year-over-year. The SLM (small language model) market is projected to grow from $0.93 billion in 2025 to $5.45 billion by 2032.
For healthcare specifically, revenue cycle is uniquely suited to task specialization. The work involves bounded decision categories, clear policy frameworks, high financial stakes, and non-negotiable audit requirements. All of these constraints actually make specialized models more attractive.
AI in healthcare is projected to grow from $26.5 billion in 2024 to nearly $188 billion within a decade. The methods that work in revenue cycle will inform how healthcare applies AI across other functions.
